Confirmed as an image problem. Downloaded the and gunzipped it then untarred it then qemu-img converted it from qcow2 to raw, losetup to make it accessible, lvs reports a new VolGroup00-LogVol00 LV present. Mounting this and ls -la'ing it shows a bunch of directories have owner/group info of '????????'. Umounted and ran xfs_repair and it spits out several thousand messages about problems it finds. Can be corrected but this is an image that should just work so it needs fixing at source and reuploading.
