See the tickets linked; IPv6 'Allow' statements don't work, because they are covered by IPv4 rules that override them. This has apparently been fixed in APR 1.4.7 and above.
Steps To ReproduceOrder Allow,Deny
Allow ipv4.add.re.ss
Allow ipv6:add:re::ss

The IPv4 address is allowed, the IPv6 is not, even if copied straight from the Apache error logs.
